The required documents for a Cyprus visa are:
You must have a valid passport or another travel document which has been issued in the past ten years. The passport must be valid for at least three more months starting from the time you intend to leave Cyprus.
It must also have at least two blank pages so the visa can be affixed.
Along with the original passport, also submit photocopies of all the used pages on your passport, including all previous visas, personal information and issuance/expiry date.
You can find the Cyprus Visa Application Form on the websites of the Cyprus Consulates and Embassies. You must complete it and sign it before submission.
If you are applying by mail, then a Notary Public must certify your signature.
For minors: Both parents or legal guardians have to sign the Application Form instead.
